4. In the list of accounts, under the minuses and pluses of the dash.
5. I do not quite understand how to exit the program, except for forced closure.

Hmm, it looks like you have the underline mode of all navigation buttons turned on. You can turn it off here iPhone - Settings - Basic - Universal Access - Button Forms.
